Dancer Consent Form

From May 25th, 2018, General Data Protection Regulation (GDPR) requires your specific consent with regards to the personal information of dancers that Peacock Dance Academy collects and uses, in-line with our Privacy Policy above. Please make yourself aware of our Privacy Policy and get in touch if you have any questions.

​If your child is under 18 years of age, parents must submit this information on their behalf. If your child is aged 13-17, they must agree with the consent being granted.

If you select 'No' to photo/video consent, your child will not be able to take part in some of our events, including the annual dance show, which is recorded.
